The following templates support consistent clinical documentation and practice development.

Consultation Documentation Template

  • Client Information: Name, contact, date of birth, relevant medical history, current medications, previous procedures, allergies
  • Structural Assessment: Bone mapping findings, facial proportions, asymmetries identified, photographs taken
  • Dynamic Assessment: Muscle behaviour, expression patterns, dynamic asymmetries noted
  • Skin Assessment: Fitzpatrick type, undertone, texture, oiliness, sensitivity
  • Design Discussion: Client preferences expressed, reference images reviewed, design recommendations made, agreed design documented
  • Colour Planning: Pigment selection, undertone considerations, predicted outcome discussed
  • Expectation Discussion: Client expectations assessed, realistic outcomes discussed, understanding confirmed
  • Consent: Risks discussed, questions answered, consent obtained

Procedure Documentation Template

  • Pre-Procedure: Strategy review, client condition, any changes from consultation
  • During Procedure: Technique used, blade/needle configuration, pigment applied, any adjustments made and reasons, client tolerance, any concerns noted
  • Post-Procedure: Immediate appearance, aftercare instructions given, follow-up scheduled

Follow-Up Documentation Template

  • Healing Progress: Appearance at follow-up, healing events reported, any concerns
  • Retention Assessment: Colour retention, stroke definition, any areas of concern
  • Client Satisfaction: Client feedback, areas of satisfaction and concern
  • Recommendations: Touch-up needs, maintenance planning, future considerations

Outcome Review Template

For practice development, review completed cases with attention to:
